1.泛硫乙胺:
泛硫乙胺是泛酸(维生素B5)的衍生物,可作为维生素的来源。泛硫乙胺及其代谢产物似乎对人体的脂肪和胆固醇代谢途径起作用。维生素B5的一个显著功能是其转化为辅酶A,辅酶A是脂肪酸代谢为细胞能量的必要因素。泛硫乙胺衍生物半胱胺还可降低产生胆固醇和甘油三酯的肝酶活性1。研究表明,在高胆固醇人群2,3和糖尿病患者4中,每天服用900-1200mg的泛硫乙胺可显著降低总胆固醇和LDL胆固醇(高达13.5%)、甘油三酯和升高HDL,尽管在低至600mg/天的剂量下也观察到对甘油三酯的显著影响5。一项控制良好的临床试验表明,当将泛硫乙胺添加到低饱和脂肪和低胆固醇的饮食中时,可进一步降低总胆固醇和LDL胆固醇6。
2.植物甾醇:

3.鱼油:

在一项对22项补充或处方鱼油制剂的研究的综述中,评估了EPA和DHA对血脂的影响,发现DHA可使甘油三酯水平降低6.8%,HDL水平升高5.9%19。虽然已知DHA会轻微提高LDL水平,但高剂量的DHA已被证明可增加LDL周转率和LDL颗粒大小(较大的LDL颗粒可能不像较小的LDL颗粒那样促进动脉粥样硬化)20。在一项严格的临床试验中,每天4g名为Icosapent ethyl(Vascepa)的EPA处方药降低了服用他汀类药物的高甘油三酯人群发生心血管事件的风险21。一项事后分析进一步发现,Icosapent ethyl组的首次血运重建程序减少了34%,总血运重建减少了36%,支架植入手术减少32%,冠状动脉旁路移植手术减少39%22。
2019年中期,美国FDA回应了一份健康请愿书,并表示不会反对omega-3脂肪酸的合格健康声明,尽管结论是“证据不一致且不确定”。FDA没有反对的合格健康声明的一种变体如下:

4.益生菌:

在一项随机对照试验中,114名胆固醇水平高且健康的参与者在六周内每天食用一种益生菌酸奶或对照酸奶,前者提供28亿CFU的微胶囊化罗伊氏乳杆菌NCIMB 30242。罗伊氏乳杆菌组的总胆固醇(9%)和LDL胆固醇(5%)水平与安慰剂酸奶组相比有所下降。高水平的ApoB-100与血管疾病相关,在罗伊氏乳杆菌组中显著降低25,26。在另一项针对127名高胆固醇健康成年人的对照临床试验中,受试者接受罗伊氏乳杆菌NCIMB 30242胶囊或安慰剂治疗9周。与安慰剂相比,服用罗伊氏乳杆菌的患者总胆固醇下降了9%以上,LDL胆固醇水平下降了11.5%以上。与安慰剂相比,补充罗伊氏乳杆菌的组中ApoB-100与ApoA-1的比例下降了9%。ApoB-100:ApoA-1比值是心血管风险的有力预测因子,尤其是在超重和肥胖个体中27,28。纤维蛋白原和hs-CRP,心血管风险的额外标志物,也相对于安慰剂显著降低29。有趣的是,后来对研究结果的分析表明,与安慰剂相比,服用益生菌的受试者胃肠道功能症状总体改善30,维生素D水平显著升高31。

对包括1600多名受试者的30项随机对照试验的分析发现,接受益生菌治疗的受试者总胆固醇平均比未接受治疗的低7.8个百分点,LDL平均降低7.3个百分点34。另一项综述得出结论,嗜酸乳杆菌特异性益生菌在降低LDL水平方面特别有效35。此外,对随机对照试验中使用的所有乳杆菌菌株的系统综述发现,乳杆菌的使用平均使总胆固醇降低约5个百分点,使LDL固醇降低约4个百分点23。对2型糖尿病人群中益生菌使用的综述36发现,血压与补充有关37。
5.益生元:
益生元是可溶性纤维的一个子集,由于其能够被肠道菌群选择性发酵,具有多种潜在的健康促进作用而受到关注。益生元纤维发酵成短链脂肪酸,如乙酸盐、丁酸盐或丙酸盐,可能会抑制肝脏中胆固醇的合成38。在人体试验中,益生元纤维素菊粉和糊精诱导血清总胆固醇(菊粉和糊糊精分别为-9%和-2%)、LDL胆固醇(糊精为-1%)和甘油三酯(菊粉为-21%)39,40。
6.辅酶Q10:
线粒体产能是一种重要的生物学作用。排列在动脉壁上的精细内皮细胞依赖于健康的线粒体功能来控制血压和血管张力。氧化或糖化低密度脂蛋白可破坏内皮线粒体功能并破坏内皮屏障,为动脉粥样硬化级联反应的启动奠定基础41,42。辅酶Q10是线粒体代谢的一个组成部分,是ATP产生的两个主要检查点之间的中间转运体。有趣的是,由于辅酶Q10是脂溶性的,它被掺入LDL颗粒中,起到抗氧化的作用。辅酶Q10水平不足会限制内皮细胞中线粒体的效率,使LDL颗粒受到氧化损伤,从而加速动脉粥样硬化的形成。具有讽刺意味的是,通常用于治疗高胆固醇的他汀类药物也会抑制血液中辅酶Q10合成43。因此,服用他汀类药物的人应始终补充辅酶Q10。
在一项随机、对照、双盲试验中,服用他汀类药物和ω-3脂肪酸的人每天添加200mg辅酶Q10可以改善一系列心血管风险因素。总胆固醇、收缩压和炎症标志物(hs-CRP和IL-6)均显著降低,而内部抗氧化防御增加,他汀类药物相关的不良反应减少44。
2018年一项针对冠状动脉疾病患者的随机临床试验综述得出结论,补充辅酶Q10可以降低总胆固醇并增加高密度脂蛋白。这项研究还发现了辅酶Q10降低LDL和Lp(a)的趋势45。此前的一项综述还发现,补充CoQ10可以减少Lp(a)46。
7.维生素E:
天然生育酚和生育三烯酚共同形成维生素E,作为脂溶性抗氧化剂已经被研究了数十年。维生素E抑制LDL颗粒氧化47,48。
α-生育酚是最主要的维生素E形式,在血液和组织中含量最高。然而,对于任何补充维生素E的人来说,确保每天都能获得足够的γ-生育酚是至关重要的。γ-生育酚最重要的好处之一是它能够通过增加一氧化氮合酶来改善内皮功能,后者是负责产生血管舒张一氧化氮的酶49。它产生这种效果的一种主要方式是吸收破坏性的活性氮类,如过氧亚硝酸盐50。事实上,γ-生育酚能够“捕获”各种活性氮类,并阻止它们对许多细胞过程的负面影响51。
人类每天补充100mg γ-生育酚可降低血管疾病的几个风险因素(如血小板聚集和低密度脂蛋白胆固醇水平)52。
8.类胡萝卜素:
类胡萝卜素是LDL颗粒中的常见成分。β-胡萝卜素是LDL中含量第二丰富的抗氧化剂;其他常见的膳食类胡萝卜素(番茄红素、叶黄素)也可能通过LDL颗粒运输53。这三种类胡萝卜素在保护LDL颗粒免受氧化损伤方面发挥着不可或缺的作用;它们的血清水平已被证明是最能预测人类LDL氧化程度的54。
类胡萝卜素也可能具有额外的降脂活性,而与它们的抗氧化潜力无关。在这方面研究得最好的是番茄红素。对12项番茄红素人体试验的分析显示,LDL胆固醇平均降低约12%55。这种作用的潜在机制是通过抑制HMG-CoA还原酶抑制胆固醇合成或提高LDL降解率56。一项研究番茄制品消费和补充番茄红素的人体干预试验的系统综述发现,这些干预措施对血脂和血管功能有有益影响57。
9.膳食纤维:
可溶性纤维包括不易消化和可发酵的碳水化合物,它们的充足摄入与降低心血管疾病的患病率有关58。当将其作为低饱和脂肪/低胆固醇饮食的一部分时,它们可以降低高胆固醇血症和糖尿病患者的LDL胆固醇通常约为5-10%,并可以降低健康个体的LDL59。可溶性的燕麦纤维、洋车前籽、果胶、瓜尔胶、大麦β-葡聚糖和壳聚糖降胆固醇特性得到了数十项对照人体临床试验的证实60-62。2015年一项对17项随机对照试验的综述得出结论,摄入β-葡聚糖显著降低了高胆固醇血症患者的总胆固醇和LDL,没有任何不良反应63。可溶性纤维通过几种潜在机制降低胆固醇60。它们可以直接结合胆汁酸或膳食胆固醇,阻止或破坏它们的吸收。它们的高粘度(液体厚度的测量)和对肠道运动的影响可能会减缓或限制大量营养素的吸收。它们还可以增加饱腹感,从而限制整体能量摄入。
10.朝鲜蓟:
朝鲜蓟具有传统的护肝和利胆作用。在刺激胆汁流动的过程中,朝鲜蓟可以帮助身体处理多余的胆固醇。体外研究表明,其作用机制可能与LDL氧化的减少有关,或其成分之一木犀草素间接抑制HMG-CoA还原酶的能力有关64。
一些随机对照试验支持朝鲜蓟提取物降低总胆固醇和/或LDL胆固醇的能力。在一项试验中,朝鲜蓟提取物(1800mg/天)持续6周可降低71名高胆固醇血症患者的总胆固醇(-9.9%)和LDL胆固醇(-16.6%),LDL或甘油三酯没有差异65。在另一项针对高胆固醇血症的试验中,与对照组相比,每天1280mm朝鲜蓟提取物持续12周可降低6.1%的总胆固醇,而LDL胆固醇、HDL胆固醇和甘油三酯的变化并不显著66。一些研究表明,补充朝鲜蓟可以提高HDL水平67,68。
11.大蒜:
大蒜的益处已经被几项人体试验证实,尤其是它支持有利血脂的能力。对健康个体或高胆固醇和甘油三酯人群食用大蒜的32项盲法对照试验进行的三项独立分析证实,大蒜显著降低了总胆固醇平均7.3mg/dl,甘油三酯平均4.2mg/dL69,70。虽然人体研究的平均胆固醇降低幅度不大,但在最初患有高胆固醇或甘油三酯(>11mg/dL)并服用提取物超过12周(11 mg/dL)的患者中,总胆固醇的降低幅度更大。
一项对39项初步试验的综合综述得出结论,在胆固醇升高的个体中,长期(大于两个月)使用大蒜制剂似乎对降低总胆固醇和LDL有效,而且副作用发生率低71。
大蒜还能降低高血压患者的收缩压和舒张压,以及血压正常的患者的收缩压。对11项大蒜安慰剂对照人体试验的回顾和分析显示,大蒜组的收缩压平均下降4.6mmHg;高血压亚组的收缩压平均下降8.4mmHg,舒张压平均下降7.3mmHg72。
12.绞股蓝:
绞股蓝在亚洲传统医学中用于治疗多种慢性疾病,包括糖尿病和炎症性疾病。它的作用部分是由于它能够激活AMPK73,74。这种影响葡萄糖代谢和脂肪储存的酶被称为“代谢主开关”,因为它控制着许多代谢途径75,76。
AMPK激活刺激肌肉中的葡萄糖摄取和β-氧化,在此过程中,脂肪酸被分解,同时减少肝脏中脂肪和胆固醇的产生73。它还可以防止氧化的LDL对血管内皮细胞的损伤77。
绞股蓝活化AMPK并影响血液和肝脏中的胆固醇水平。一项针对肥胖小鼠的研究表明,补充绞股蓝八周可以减轻体重,改善葡萄糖代谢和胆固醇水平。与对照组相比,用150mg/kg(成人约为900mg)或300mg/kg(成年约为1800mg)提取物处理的小鼠的总胆固醇分别降低了14.2%和7.1%73。在一项为期12周的绞股蓝安慰剂对照试验中,与安慰剂相比,全身脂肪和体脂百分比均显著降低76。
13. 橙皮苷:
橙皮苷和相关黄酮类化合物存在于多种植物中,尤其是柑橘类水果的果皮中78,79。橙皮苷的消化会产生一种称为橙皮素(Hesperetin)的化合物以及其他代谢产物。这些化合物是强大的自由基清除剂,并具有抗炎、增加胰岛素敏感性和降脂活性80,81。动物和体外研究结果表明,橙皮苷对血糖和脂质水平的积极作用可能部分与AMPK通路的激活有关82,83。越来越多的证据表明,橙皮苷可能有助于预防和治疗与衰老相关的许多慢性病80。
橙皮苷可以部分通过激活AMPK信号通路来预防糖尿病及其并发症。巧合的是,二甲双胍,一种主要的糖尿病药物,也激活AMPK通路。在一项针对24名糖尿病参与者的为期六周的随机对照试验中,每天补充500 mg橙皮苷可以改善血糖控制,提高总抗氧化能力,减少氧化应激和DNA损伤84。使用尿橙皮苷作为膳食橙皮苷的标志物,另一组研究人员发现,与摄入水平最低的人相比,摄入橙皮苷水平最高的人在4.6年内患糖尿病的风险低32%85。
在一项随机对照试验中,24名患有代谢综合征的成年人每天服用500mg橙皮苷或安慰剂治疗三周。洗脱期后,用橙皮苷重复试验,安慰剂分配颠倒。橙皮苷治疗改善了内皮功能,这可能是其有益于心血管系统的重要机制之一。相对于安慰剂,补充橙皮苷还导致炎症标志物hs-CRP的中位水平降低33%,总胆固醇、ApoB和血管炎症标志物的水平显著降低83。在一项针对有血管功能障碍证据的超重成年人的随机对照试验中,连续六周每天补充450 mg橙皮苷可降低血压并降低血管炎症标志物86。在另一项随机对照试验中,包括75名心脏病发作患者,他们在四周内每天接受600mg橙皮甙或安慰剂治疗,服用橙皮苷的人在HDL胆固醇、血管炎症标志物、脂肪酸和葡萄糖代谢方面有显著改善87。
14. 绿咖啡(绿原酸):
未经烘焙的咖啡豆提取物或生的绿咖啡豆提取物已被用于促进心血管和代谢健康。绿咖啡含有高水平的多酚,称为绿原酸(CGAs),这些多酚被认为是咖啡中重要的健康促进化合物。然而,传统的咖啡烘焙方法通常会降低多酚水平88。临床前研究表明,CGA可能通过促进胆固醇7α羟化酶(CYP7A1)的表达增加来帮助调节胆固醇水平,CYP7A1是负责催化胆固醇转化为胆汁酸的第一步的酶89。在小鼠模型中,降低血浆总胆固醇和LDL胆固醇水平,促进巨噬细胞排出胆固醇,这一过程有助于防止动脉粥样硬化斑块的形成。它还降低了甘油三酯和一些炎症生物标志物90。
在健康人群和心脏代谢疾病患者中进行的大量临床研究已经证明了绿咖啡提取物对脂质水平以及其他心脏代谢标记物的益处91。在一项对14项随机对照试验(包括766名参与者)的荟萃分析中,剂量范围为90至6000mg/天(提供13.5至1200mg/天CGA)的绿咖啡提取物达16周可降低血清总胆固醇。在持续八周以上的研究中,LDL也显著降低92。在一项对包括992名参与者在内的27项随机对照试验的荟萃分析中,类似剂量(200至2000mg/天)的绿咖啡提取物可持续24周,降低血清甘油三酯,提高高密度脂蛋白水平93。绿咖啡提取物似乎对葡萄糖和胰岛素水平、体重和血压等也有积极影响94-96。
15. 穆库树胶:
这是一种Commiphora mukul树的树脂,在阿育吠陀医学中使用悠久。Guggulipid是穆库树胶的脂质提取物,含有植物甾醇(guggulsterones E和Z),这是一种拟议的生物活性成分97。质量不一致的证据表明,补充可能有助于改善脂质状况98-101。不过,尚需要更好的设计试验来阐明功效。
16. 茶多酚:
多酚是一种多种多样的植物营养素,在饮食中无处不在。绿茶和红茶都含有多酚。在几项研究的分析中,绿茶儿茶素(EGCG)表现出显著的胆固醇(LDL)降低(四项研究平均约为9mg/dl)102。一项荟萃分析证实了绿茶对总胆固醇、LDL以及收缩压的有益作用103。在有患糖尿病风险的肥胖女性中,绿茶与糖尿病药物二甲双胍的头对头比较发现,绿茶在降低总胆固醇和LDL方面表现更好104。随后,一项对47名轻度高胆固醇症的日本男性和女性进行的红茶提取物研究表明,三个月后,总胆固醇降低了8%,LDL胆固醇降低了13%105。在一系列临床前研究中,红茶中的茶黄素有效地降低了总胆固醇、低密度脂蛋白和甘油三酯106。
17. 石榴:
石榴现在被广泛认为是一种具有许多健康益处的超级水果;已经对石榴汁或石榴提取物进行了数十项安慰剂对照临床试验。在脂质管理方面,石榴的功效可与极少数的天然化合物相媲美。石榴中高浓度的多酚(尤其安石榴苷/Punicalagins)使其成为抑制LDL氧化的理想成分107,108。
石榴多酚的摄入显著降低了胆固醇水平升高的受试者的总胆固醇和LDL胆固醇浓度,同时保持了HDL水平109。石榴还抑制了对氧化LDL的免疫反应性,这一机制有望限制内膜斑块的形成110。事实上,这正是一项关于石榴消费的长期研究所表明的。受试者接受石榴汁或安慰剂治疗三年;在安慰剂组中,研究开始一年后,颈动脉内膜-中膜厚度(cIMT;衡量动脉粥样硬化的指标)增加了9%,而石榴组的cIMT减少了30%。与安慰剂相比,石榴还显著降低了氧化LDL浓度,提高了血清抗氧化活性,同时降低了血压。此外,石榴使对氧合酶-1(PON-1)的活性几乎翻了一番,PON-1是一种优化HDL功能并保护脂质免受氧化损伤的抗动脉粥样硬化酶111。两组都继续进行标准治疗,可能包括他汀类药物、抗高血压药等。
在一项随机对照临床试验中,患有代谢综合征的女性服用富含多酚的石榴汁或水。研究发现,接受果汁的女性红细胞膜中的氧化应激标志物水平明显较低,健康的单不饱和脂肪酸水平较高。果汁组的血浆促炎脂肪花生四烯酸水平降低112。在另一项随机安慰剂对照试验中,食用石榴汁可降低血液透析患者的动脉粥样硬化风险因素113。在一项针对48名超重或肥胖参与者的试验中,每天1000mg石榴提取物可改善总胆固醇、LDL胆固醇,HDL胆固醇水平以及代谢健康和炎症的其他几种标志物114。
18. 姜黄素:
姜黄素是姜黄根的主要活性成分,在心血管疾病中具有多种保护作用,可能减少氧化应激、炎症以及平滑肌细胞和单核细胞的增殖。95项小型人体试验揭示了姜黄素对降低脂质过氧化的作用115,116和血浆纤维蛋白原117,二者都是动脉粥样硬化进展的因素118。姜黄素也可以通过增加LDL受体的产生来降低血清胆固醇119,120。
但是,尽管在动物模型中取得了成功,关于姜黄素抗高胆固醇血症作用的人类数据存在矛盾。一项针对10名健康志愿者的小型研究显示,在连续7天每天使用500mg姜黄素时,脂质氧化产物(-33%)和总胆固醇(-12%)显著降低,同时HDL胆固醇(29%)增加121。在随后的两项研究中,低剂量姜黄素在急性冠状动脉患者中显示出降低总胆固醇和LDL的非显著趋势122,而高剂量姜黄素(1-4g/天)的总胆固醇、LDL和HDL胆固醇表现无显著增加123。
对七项随机对照试验的综合分析得出结论,姜黄和姜黄素在降低LDL胆固醇和甘油三酯方面有效,没有任何严重的副作用124。在一项针对2型糖尿病患者的研究中,与安慰剂相比,补充姜黄可降低体重指数、总胆固醇和甘油三酯125。一项涉及代谢综合征患者的类似研究发现,姜黄素对脂质参数(降低LDL和增加HDL)有有益的影响126。
19.红曲米:
红曲米是由红曲酵母发酵而成的传统米制品。酵母产生的代谢产物(Monacolins,莫那可林)是天然存在的HMG-CoA还原酶抑制剂(其中一种,Monacolin k在化学上与洛伐他汀相同)127。一项对包括近10,000名受试者在内的93项随机试验的全面综述表明,红曲米的商业制剂降低了总胆固醇、LDL胆固醇、甘油三酯,以及HDL胆固醇的增加128。一项多中心、良好对照的试验中,将一种名为“Xuezhikang(血脂康)”的红曲米提取物与安慰剂进行了比较,结果表明LDL降低了27%129。一项对22项临床试验的综述表明,在冠心病的常规治疗中加入血脂康,可以降低死亡和心脏病发作的风险130。

21.余柑子

症状
高胆固醇引起症状是罕见的,一般只有通过血液检测才能发现。并发症
高胆固醇会增加动脉粥样硬化的危险,后者可阻止血液流动、导致如下症状:此外,一些高胆固醇患者在其肌腱、眼睛下面或眼睛里也可能有胆固醇沉积。
疗法

调整饮食与生活方式

有助于防控高胆固醇的营养和草本补充剂,主要包括如下:
1.泛硫乙胺:
泛硫乙胺是泛酸(维生素B5)的衍生物,可作为维生素的来源。泛硫乙胺及其代谢产物似乎对人体的脂肪和胆固醇代谢途径起作用。维生素B5的一个显著功能是其转化为辅酶A,辅酶A是脂肪酸代谢为细胞能量的必要因素。泛硫乙胺衍生物半胱胺还可降低产生胆固醇和甘油三酯的肝酶活性1。研究表明,在高胆固醇人群2,3和糖尿病患者4中,每天服用900-1200mg的泛硫乙胺可显著降低总胆固醇和LDL胆固醇(高达13.5%)、甘油三酯和升高HDL,尽管在低至600mg/天的剂量下也观察到对甘油三酯的显著影响5。一项控制良好的临床试验表明,当将泛硫乙胺添加到低饱和脂肪和低胆固醇的饮食中时,可进一步降低总胆固醇和LDL胆固醇6。
2.植物甾醇:
植物甾醇是在植物中发现的类固醇化合物,其功能与动物体内的胆固醇类似(即作为植物细胞膜的成分和植物激素的前体)7。与胆固醇一样,它们可以作为游离分子或甾醇酯存在。甾醇酯具有更高的活性和更好的脂溶性,这允许更低的有效剂量(2-3g/天,而未酯化甾醇为5-10g/天)8。甾醇本身从饮食中吸收很差,但由于其与胆固醇的化学相似性,被认为与胆固醇在肠道中竞争吸收,它具有降低LDL水平的净作用9。甾醇还可以减少肝脏中胆固醇的产生,减少VLDLs的合成,增加LDL的颗粒大小,并增加血液中对LDL的摄取10,11。HDL通常不受甾醇摄入的影响12。

3.鱼油:

2019年中期,美国FDA回应了一份健康请愿书,并表示不会反对omega-3脂肪酸的合格健康声明,尽管结论是“证据不一致且不确定”。FDA没有反对的合格健康声明的一种变体如下:

4.益生菌:
益生菌因其在调节免疫活性和减少全身炎症方面的作用而越来越受到认可,其降低LDL胆固醇和心血管风险的能力也引起了人们的兴趣。一项研究综述发现23,罗伊乳杆菌NCIMB 30242可降低总胆固醇和LDL胆固醇水平以及炎症标志物24。
在一项随机对照试验中,114名胆固醇水平高且健康的参与者在六周内每天食用一种益生菌酸奶或对照酸奶,前者提供28亿CFU的微胶囊化罗伊氏乳杆菌NCIMB 30242。罗伊氏乳杆菌组的总胆固醇(9%)和LDL胆固醇(5%)水平与安慰剂酸奶组相比有所下降。高水平的ApoB-100与血管疾病相关,在罗伊氏乳杆菌组中显著降低25,26。在另一项针对127名高胆固醇健康成年人的对照临床试验中,受试者接受罗伊氏乳杆菌NCIMB 30242胶囊或安慰剂治疗9周。与安慰剂相比,服用罗伊氏乳杆菌的患者总胆固醇下降了9%以上,LDL胆固醇水平下降了11.5%以上。与安慰剂相比,补充罗伊氏乳杆菌的组中ApoB-100与ApoA-1的比例下降了9%。ApoB-100:ApoA-1比值是心血管风险的有力预测因子,尤其是在超重和肥胖个体中27,28。纤维蛋白原和hs-CRP,心血管风险的额外标志物,也相对于安慰剂显著降低29。有趣的是,后来对研究结果的分析表明,与安慰剂相比,服用益生菌的受试者胃肠道功能症状总体改善30,维生素D水平显著升高31。

对包括1600多名受试者的30项随机对照试验的分析发现,接受益生菌治疗的受试者总胆固醇平均比未接受治疗的低7.8个百分点,LDL平均降低7.3个百分点34。另一项综述得出结论,嗜酸乳杆菌特异性益生菌在降低LDL水平方面特别有效35。此外,对随机对照试验中使用的所有乳杆菌菌株的系统综述发现,乳杆菌的使用平均使总胆固醇降低约5个百分点,使LDL固醇降低约4个百分点23。对2型糖尿病人群中益生菌使用的综述36发现,血压与补充有关37。
5.益生元:
益生元是可溶性纤维的一个子集,由于其能够被肠道菌群选择性发酵,具有多种潜在的健康促进作用而受到关注。益生元纤维发酵成短链脂肪酸,如乙酸盐、丁酸盐或丙酸盐,可能会抑制肝脏中胆固醇的合成38。在人体试验中,益生元纤维素菊粉和糊精诱导血清总胆固醇(菊粉和糊糊精分别为-9%和-2%)、LDL胆固醇(糊精为-1%)和甘油三酯(菊粉为-21%)39,40。
6.辅酶Q10:
线粒体产能是一种重要的生物学作用。排列在动脉壁上的精细内皮细胞依赖于健康的线粒体功能来控制血压和血管张力。氧化或糖化低密度脂蛋白可破坏内皮线粒体功能并破坏内皮屏障,为动脉粥样硬化级联反应的启动奠定基础41,42。辅酶Q10是线粒体代谢的一个组成部分,是ATP产生的两个主要检查点之间的中间转运体。有趣的是,由于辅酶Q10是脂溶性的,它被掺入LDL颗粒中,起到抗氧化的作用。辅酶Q10水平不足会限制内皮细胞中线粒体的效率,使LDL颗粒受到氧化损伤,从而加速动脉粥样硬化的形成。具有讽刺意味的是,通常用于治疗高胆固醇的他汀类药物也会抑制血液中辅酶Q10合成43。因此,服用他汀类药物的人应始终补充辅酶Q10。

2018年一项针对冠状动脉疾病患者的随机临床试验综述得出结论,补充辅酶Q10可以降低总胆固醇并增加高密度脂蛋白。这项研究还发现了辅酶Q10降低LDL和Lp(a)的趋势45。此前的一项综述还发现,补充CoQ10可以减少Lp(a)46。
7.维生素E:
天然生育酚和生育三烯酚共同形成维生素E,作为脂溶性抗氧化剂已经被研究了数十年。维生素E抑制LDL颗粒氧化47,48。

8.类胡萝卜素:

类胡萝卜素也可能具有额外的降脂活性,而与它们的抗氧化潜力无关。在这方面研究得最好的是番茄红素。对12项番茄红素人体试验的分析显示,LDL胆固醇平均降低约12%55。这种作用的潜在机制是通过抑制HMG-CoA还原酶抑制胆固醇合成或提高LDL降解率56。一项研究番茄制品消费和补充番茄红素的人体干预试验的系统综述发现,这些干预措施对血脂和血管功能有有益影响57。

18. 姜黄素:
